Medication Management Service at UPMC St. Margaret Family Health Centers

Clinical Pharmacists provide drug therapy management in collaboration with referring physicians. Common chronic disease state management areas include diabetes, hypertension, and hyperlipidemia. Patients on chronic anticoagulation therapy and multiple medications are also commonly seen. Providers work closely with social work in helping patients afford and manage their medication regimens.

Services

Medication Therapy Management Services to a wide-range of patients in their communities. Clinical Pharmacists also participate in a weekly collaborative diabetes education class for patients.
