Patients who are chronically followed by UPMC physicians or are discharged from UPMC Presbyterian or Montifiore Hospitals.  Patients vary in age, social and health status.

The University of Pittsburgh Medical Center (UPMC) Anticoagulation Service

This anticoagulation service assists UPMC physicians in the management of patients prescribed warfarin therapy. Through patient education, intensive laboratory monitoring, patient follow-up, and assessment of patient response, adverse effects of warfarin therapy are minimized in an attempt to maximize therapeutic benefit.

Services Provided

Clinical Pharmacists provide drug therapy management through a collaborative agreement and protocol with participating physicians for patients on warfarin (Coumadin) therapy.  Currently over 900 patients are enrolled.

Student/Resident Participation

P4 students on experiential rotation
Primary Care and Pharmacy Practice Residents participate in longitudinal and block rotations
