Falk Pharmacy Community Care – Medication Therapy Management Service

Falk Pharmacy Medication Therapy Management (MTM) Service is designed to provide medication therapy management through personalized, one-on-one time with a specially trained pharmacist.  A scheduled visit occurs in a private setting, away from the prescription pick-up area.  The pharmacist will work with the patient and his/her physician to ensure appropriate medication use for the patient. 

During the MTM visit, the pharmacist will discuss and review the patient’s medication use and medical history. This discussion will ensure patients are taking medications properly, and address all of the individual’s medication and health care concerns.  The pharmacist will provide education on all medications; including prescription, non-prescription and dietary supplements; medical conditions and general health and wellness. The pharmacist will also review each medication for side effects, interactions or available lower cost alternatives.  The patient’s physician will be provided an updated list of the patient’s medications as well as a visit summary outlining any patient concerns and recommendations.

Services Provided

Medication Therapy Management including specialized services for diabetes management, smoking cessation and vaccinations.

Research

Current research includes identification of documentation of services for reimbursement; ideal marketing strategies of services with physician groups; and evaluation of patient compliance impact on therapeutic outcomes

Student/Resident Participation

Several different levels of learners are exposed to Falk Pharmacy MTM Services; including P2 and P4 experiential pharmacy students; PGY 1 Pharmacy Practice and Community Pharmacy Practice Residents; and PGY 2 Ambulatory Care Pharmacy Residents.
