Clinical Rotation: Cardiothoracic/ Surgical ICU

Preceptor: Kimberly Maslonek, PharmD

This rotation will expose residents to the practice of pharmacotherapy within the CTICU/ SICU setting. Residents will be expected to work closely with the health care team to provide timely, safe and effective pharmacy services in an academic health system environment. Residents will complete the objectives below by attending daily clinical rounds, reviewing and assessing patient medication profiles and communicating pharmacotherapy plans with health care providers and the patients.

The objectives of the rotation are as follows:

  1. Goal: Demonstrate ownership of and responsibility for the welfare of the patient by performing all necessary aspects of the medication-use system.
  2. Goal: As appropriate, establish collaborative professional relationships with members of the health care team.
  3. Goal: Place practice priority on the delivery of patient-centered care to patients.
  4. Goal: As appropriate, establish collaborative professional pharmacist-patient relationships.
  5. Goal: Collect and analyze patient information.
  6. Goal: When necessary, make and follow up on patient referrals.
  7. Goal: Design evidence-based therapeutic regimens.
  8. Goal: Design evidence-based monitoring plans.
  9. Goal: Recommend or communicate regimens and monitoring plans.
  10. Goal: Implement regimens and monitoring plans.
  11. Goal: Evaluate patients’ progress and redesign regimens and monitoring plans.
  12. Goal: Communicate ongoing patient information.
  13. Goal: Document direct patient care activities appropriately.
  14. Goal: Provide effective medication and practice-related education, training, or counseling to patients, caregivers, health care professionals, and the public.
  15. Goal: Use information technology to make decisions and reduce error.
  16. Goal: Understand unique aspects of providing evidence-based, patient-centered medication therapy management with interdisciplinary teams in the managed care environment.
  17. Goal: Participate in the management of medical emergencies.
  18. Goal: Identify a core library, including electronic media, appropriate for a specific practice setting.
  19. Goal: Design and deliver programs that contribute to public health efforts. .
  20. Goal: Provide concise, applicable, comprehensive, and timely responses to requests for drug information from patients, health care providers, and the public.
  21. Goal: Use approaches in all communications that display sensitivity to the cultural and personal characteristics of patients, caregivers, and health care colleagues.
  22. Goal: Communicate effectively.
  23. Goal: Balance obligations to oneself, relationships, and work in a way that minimizes stress.
  24. Goal: Manage time effectively to fulfill practice responsibilities.
